Repair Process
To use our repair services, please complete
the printable*
Shoe Repair Form and mail with your
shoes to our repair center. Simply
fill out the printable Shoe Repair Form
requesting the work to be done. Our
experienced shoe repairmen will complete
your repairs as you have requested. Your
repair item(s) will be shipped back to you
within 48 hours of when we receive them via
UPS Ground.
All repair items must be paid for by credit
card. If you are unsure of what
needs to be done to repair your shoes,
please give us a call at 801-372-271 or
toll-free at 1-800-796-1910 to discuss your
options. You may also want to email us
pictures of your shoes so we can better
determine what needs to be done. You
can email images to us at customerservice@modernshoe.com.
